- If your school has not yet provided you this information, you should submit the COPPA Disclosure Request Form to your school’s principal and/or child’s teacher.
- Once the disclosure form is returned with this information, you should submit the COPPA Opt-Out Form to each online vendor or service provider (and cc: your school) using the contact information provided by your child’s teacher or school;
- Or you can submit the COPPA Opt-Out Form to your school/district directing them to contact the vendors or service providers on your behalf. (Note: COPPA does not specify that your school/district must provide you assistance when contacting vendors or service providers; your request may be denied in which case you can use the form in #2 to contact the vendors or service providers directly.)

- Please inform me of all the online contractors, vendors or other third parties that have received my child’s personally identifiable information or will do so over the course of this school year.
- Please inform me of which personally identifiable data each of them has received, or will in the future, and under what privacy and security protections they will be stored or disclosed to other third parties.
- Please inform me of all online programs that my child is currently participating in at school, or will be participating in over the course of this school year, including online testing.
- Please provide me with a copy of the privacy policy and terms of service of each of the above vendors or contractors listed under #1 and #3. [Note: If you are unable to produce copies because the operator failed to provide it then you and/or the vendor may be in violation of Children’s Online Privacy Protection Act (COPPA).]
- Please provide me with the contact information, including the email addresses of all of those vendors or providers listed under #3, to enable me to determine what personal data are currently being collected for my child or will be in the future, how this data is being used, with whom it may be shared, and so that I can order it deleted and/or opt out of further collection if I so choose, which are my rights under COPPA.

The Children’s Online Privacy Protection Act, or COPPA, is a federal law that allows parents to control what information is collected online from their children under the age of 13.
